Perry Lutheran Homes is looking for caring Dementia Practitioners to take great care of our loved residents in our memory care neighborhoods!

Position and Shift Offerings:

Full Time (CNA/CMA):

  • 2PM-10PM

Part-Time or PRN (CMA/CNA):

  • 6AM-2PM
  • 2PM-10PM
  • 10PM-6AM
Shift Differential Pay: 10% more for 2pm-10pm, 5% more for 10pm-6am

  • Assist residents with daily living activities focusing on safety and compassion
  • Become a valuable team-member working together to create positivity
  • Demonstrate interpersonal effectiveness by building meaningful relationships
  • Use critical thinking skills to constantly improve and innovate
  • Read and contribute to shift reports and care notes to ensure continuity of care.
  • Greet visitors and direct them to the right room.
  • Respond to emergencies situations by following Perry Lutheran Homes policies and procedures.
  • If a medication administration class is successfully completed by employee, administer medications per the MAR.
  • Report to Nurse or Nursing Leadership if any signs of community member abuse or witnessed misappropriation.
  • Report to Memory Care Coordinator when not able to work or when in overtime.
  • Complete Health Stream, in-person, and any other required training according to state and federal regulation.
  • Assist to provide community members with a safe, home-like environment.
  • Perform other duties as assigned or delegated by nurse or memory care coordinator verbally or in writing.

Requirements:

  • Functional literacy in written and spoken English.
  • High school graduate or equivalent.
  • Certified Nursing Assistant with dementia caregiving experience.
  • Medication administration or Certified Medication Aide experience with proof of Department of Health Services approved medication administration program listing on state registry.
  • Experience with electronic records and ability to write reports and correspondence (Point Click Care experience preferred).
  • Physical and emotional capacity, education and experience to respond to community member needs.
  • Physical demands such as; standing, walking, sitting, bending, climbing stairs, hearing, seeing, kneeling, lifting up to 35 pounds or more.
  • Ability to effectively communicate with confidence, clarity, precision, and professionalism.
  • Flexibility to meet the twenty-four hour care needs of community members.
  • Must be at least 18 years of age.
  • Basic computer knowledge and skills (Excel, Word, Outlook, Google Docs, and more).
  • Ability to connect and build rapport with residents, resident families, and staff.
  • Demonstrates great problem-solving skills, communication skills, and time management skills.
